Terlipressin

A slow-release vasopressin prodrug that constricts the splanchnic circulation, used to improve kidney function in hepatorenal syndrome and to stop variceal bleeding in cirrhosis.

FDA approved in 2022 on the CONFIRM trial, which showed better reversal of hepatorenal syndrome than placebo but also a clear excess of respiratory failure and no survival benefit. It has been standard care in Europe for far longer.

How it works

Terlipressin is lysine-vasopressin with three glycyl residues attached to the N-terminus. Endothelial peptidases cleave those glycines over hours, so a single injection produces a slow, sustained release of active lysine-vasopressin rather than a spike. The active moiety is relatively V1a-selective, so the dominant effect is arteriolar constriction in the splanchnic bed. In cirrhosis, splanchnic vasodilation is what drives the effective arterial underfilling that activates the renin-angiotensin system and shuts down renal perfusion; reversing it raises mean arterial pressure and renal blood flow, which is how terlipressin reverses hepatorenal syndrome. In variceal bleeding the same splanchnic constriction cuts portal inflow and portal pressure.

Mechanism in depth

Terlipressin is a genuinely elegant piece of design and understanding it explains both the benefit and the boxed warning. The parent molecule has low intrinsic receptor activity; the three glycines have to come off first, and endothelial and tissue peptidases do that gradually, so a 2-minute bolus produces a plateau of lysine-vasopressin rather than a spike. The half-lives in the label make this explicit: the prodrug clears with a 0.9 hour half-life while the metabolite's is 3 hours, and the metabolite's clearance and volume of distribution are more than an order of magnitude larger. Lysine-vasopressin is V1a-preferring, and the mesenteric and splanchnic arterioles are where V1a density is highest, which is why the drug acts where it does. In cirrhosis the pathophysiology is splanchnic arterial vasodilation causing effective arterial underfilling, which activates the renin-angiotensin-aldosterone system and sympathetic outflow and shuts down renal cortical perfusion. Constricting the splanchnic bed raises mean arterial pressure, refills the effective arterial volume, switches off the vasoconstrictor reflexes acting on the kidney, and renal perfusion recovers. That is why creatinine falls over days rather than hours, and why albumin is co-administered: you need the volume as well as the tone. The respiratory failure that carries the US boxed warning is the same coin's other face. Raising afterload and shifting volume centrally in a patient with tense ascites, a low cardiac reserve and often subclinical cirrhotic cardiomyopathy produces pulmonary oedema, and in the CONFIRM population it was common enough to offset the renal benefit.

What usually goes wrong

The boxed warning is the honest summary of what goes wrong: patients drown. Anyone with baseline hypoxia, volume overload or ACLF grade 3 has an unfavourable risk-benefit and the label says so. Assessing oxygenation before every dose sounds excessive until you realise the deterioration is often over hours. Second, ischaemic complications: skin necrosis at and well beyond injection sites is well documented, as are digital, cardiac and intestinal ischaemia, and the patients most likely to develop them are the ones with existing vascular disease nobody asked about. Third, rapid sodium correction. Cirrhotic patients are chronically hyponatraemic; terlipressin can drop sodium further and then recovery can overcorrect, and osmotic demyelination is a real reported outcome. Fourth, continuing past day 7 in a non-responder. Fifth, treating this as a bridge to nothing: terlipressin buys renal function, and if there is no transplant plan or no reversible precipitant, the benefit evaporates.

Titration ladder

Metabolism
Tissue peptidases cleave the three N-terminal glycyl residues, liberating pharmacologically active lysine-vasopressin. That cleavage is the rate-limiting step and it is why one bolus behaves like a slow infusion.
Elimination
Almost entirely metabolic. Less than 1 percent of terlipressin and less than 0.1 percent of lysine-vasopressin appears unchanged in urine in healthy subjects.

What to expect, and when

Mean arterial pressure rises within an hour of the first bolus and portal pressure falls in the same window, which is why the variceal bleeding effect is immediate. Renal recovery is slow: creatinine improvement in hepatorenal syndrome takes 3 to 14 days, and the day-4 assessment point exists precisely because judging earlier is meaningless. Respiratory deterioration, when it happens, typically appears in the first several days. The whole course is capped at 14 days.

Stacking and comparisons

Albumin is not an optional add-on, it is half the therapy: terlipressin supplies arterial tone and albumin supplies the volume and oncotic pressure that make the tone useful, and every positive trial used both. The catch is that albumin volume is also what drives the pulmonary oedema, so the modern practice is to be less generous with albumin than the original protocols were and to watch central volume status carefully. Non-selective beta-blockers are usually held during an acute hepatorenal episode because they blunt the cardiac output response. Do not combine with vasopressin or with somatostatin analogues; guidelines choose one vasoactive agent. Nephrotoxins, NSAIDs and contrast come off the chart entirely.

Against noradrenaline for hepatorenal syndrome: noradrenaline plus albumin achieves broadly similar reversal rates in several randomised comparisons, costs almost nothing, and is what much of the world uses, though it requires a central line and an intensive care bed. That comparison is the single most important practical point about this drug. Against midodrine plus octreotide: a much weaker oral option used where intravenous therapy is not feasible, with clearly inferior reversal rates. Against somatostatin and octreotide in variceal bleeding: terlipressin is the only one of the three with a mortality signal in meta-analysis, but it also causes the most ischaemia. Against vasopressin: same receptor family, but terlipressin's slow-release prodrug design and splanchnic selectivity make it usable outside intensive care in a way vasopressin is not.

Rough cost

Not a monthly drug. Terlivaz is priced per vial as an inpatient product and a full 14-day US course has been reported in the tens of thousands of dollars, but I did not source a figure in this session. In Europe, where generic Glypressin has been available for decades, the cost is a small fraction of that.

Genuinely uncertain

  • Plasma protein binding is not stated for either terlipressin or lysine-vasopressin in the label.
  • The 1-day time to steady state is an inference from 6-hourly dosing of a molecule whose active metabolite has a 3 hour half-life, not a quoted label figure.
  • CONFIRM enrolled around 300 patients over roughly 14 days of treatment with 90-day follow-up, but I did not confirm the numbers, so both are null.
  • Numeric V1a versus V2 selectivity ratios for lysine-vasopressin were not resolved this session.
